I am a senior social work specialist.  I have provided Senior Needs Assessments for people 65 yrs and older, in nursing home and rehabilitation center settings, for 20 years.  For my patients in the rehabilitation center setting: I initiate home health care services, provide information on private duty home care agencies, provide information on Medicare insurance coverage, assist with Medicaid applications, assist with obtaining durable medical equipment, and provide assistance in the transition of moving the patient to a new living environment.
